Case of Thrombosis with Thrombocytopenia following mRNA COVID-19 Vaccine

Probably the first case of thrombosis with thrombocytopenia following the administration of an mRNA COVID-19 vaccine has been reported, in a letter to Annals of Internal Medicine.

Authors have mentioned what according to them seems to be vaccine-induced thrombosis with thrombocytopenia (VITT) or thrombocytopenia with thrombosis syndrome (TTS) in a 65-year-old man who was administered a second dose of the Moderna vaccine 10 days prior to the onset of symptoms.

The patient came with 1 week of discomfort in both lower extremities, intermittent headaches, and dyspnea since 2 days. There appeared to be no known previous heparin exposure. The patient had developed large, bilateral, acute pulmonary emboli with right ventricular strain as well as acute deep venous thromboses in both lower extremities. He had severe thrombocytopenia and an inferior vena cava filter was placed. He was given two doses of intravenous immunoglobulin and 40 mg of dexamethasone intravenously for 4 days for possible immune-mediated thrombocytopenia. Unfractionated heparin therapy was started after platelet transfusions increased the platelet levels. Three 3 days later, he developed acute gluteal hematoma; heparin had to be withdrawn. Thrombocytopenia continued and the anti-platelet factor 4/heparin IgG assay was found to be strongly positive. After a span of 12 hours, the patient developed acute encephalopathy. CT scan revealed cerebral venous sinus thrombosis. Lower-extremity deep venous thromboses progressed and an upper-extremity deep venous thrombosis also developed. Blood cultures revealed methicillin-sensitive Staphylococcus aureus; antibiotics were started. Bivalirudin treatment and plasmapheresis was given.
